Your First Visit

Before Your First Appointment

After scheduling your new patient consultation, you will receive an email with your username and password to access our Patient App. Please login to your Patient App and complete the patient information forms 5 days prior to your appointment. Additionally, we ask that you have your primary care physician and/or OBGYN release your medical records to our office. This information is necessary for us to be able to make an accurate assessment of your condition.

What to Bring to Your First Appointment 

On the day of your first appointment, please arrive 15 minutes before your scheduled appointment and bring the following items: 

  • Insurance card
  • Photo ID
  • Insurance co-payment or self-payment 

What You Will Accomplish During Your First Appointment 

During your first appointment, you will meet with one of our fertility specialists for a detailed discussion of your medical history. Based on this conversation, your provider may perform a physical examination or recommend certain fertility testing or treatment procedures. You will have an opportunity to ask any questions that you may have and to review the options available to you.

What Happens After

After your initial consultation, we’ll work together to figure out your treatment timeline and discuss what you can expect at each step. We’ll maintain clear communication throughout your journey to ensure you understand the process and are ready for each step.
