At the Center for Reproductive Medicine, we are committed to providing each of our patients with the highest quality of care possible. When you become one of our patients, we immediately begin working to determine the source of your fertility problems and the best plan of action to overcome them. Your first appointment at our fertility clinic in the Orlando, Florida area will focus on gathering information and familiarizing you with our practice.
Your First Appointment
Prior to your first appointment, you will need to complete our new-patient information forms and have your medical records forwarded to our fertility clinic. This information is necessary for us to be able to make an accurate assessment of your condition. Please print the forms below, fill them out, and bring them with you to your appointment or fax them to our office in advance. You can have your medical records sent to us by contacting your primary care physician and/or Ob/Gyn.
When you arrive at our fertility clinic for your appointment, additional forms may need to be completed. You will then meet with one or more of our physicians as well as members of our nursing staff. We will discuss your medical history in detail and may perform a physical examination. Based on our evaluation of your medical information, we may recommend certain fertility testing or treatment procedures. You will have an opportunity to ask any questions that you may have and to review the options available to you.
If you make the decision to proceed, we will schedule the necessary follow-up visits before you leave the fertility clinic. It is best to make these appointments as far in advance as possible.
